Rehabilitation Case Manager - Brain Injury

ReMed, a Collage Rehabilitation Partners is looking for a full time Rehabilitation Case Manger to join our outstanding team in our Pittsburgh, PA location. Collage Rehabilitation Partners is a growing organization dedicated to excellence, honesty and ethics in the treatment of our clients and each other. For more than 35 years, we’ve developed programs to meet the diverse needs of clients with moderate to severe traumatic and acquired brain injuries. We provide treatment for post-concussion syndrome, stroke, memory loss, neurobehavioral issues and accident or sports-related head injuries. Our CARF accredited, post acute residential, vocational and outpatient programs are offered in the Philadelphia, Pittsburgh regions of Pennsylvania, New Jersey, Louisiana and Maryland, Georgia, North Carolina, Virginia, Texas, Utah, Colorado, and California

Responsibilities

This position is responsible for ensuring the implementation of the clients’ treatment plan(s), from admission through discharge and follow-up. This role coordinates schedules, therapies and other appointments/services, and is responsible for regular contact with families and funders, as well as ongoing internal report writing, clinical documentation and facilitating funding issues as needed. The individual serving in this position acts as a client advocate and is responsible for client treatment reports and chart documentation.

Duties & Responsibilities

Demonstrates ability to support clients in difficult situations (e.g. refusal of activities, medications, and strategies)

Works directly with the client and provides minimum to maximum assistance and/or supervision to promote the client’s success in achieving identified treatment goals.

Responsible for managing assigned cases according to policies, procedures and guidelines in the RCM manual and Goal Writing manual.

Leads team meetings and interfaces with treatment team to establish rehabilitation goals and coordinates treatment necessary to support client’s treatment plan. Assists team in remaining focused on goals and discharge plan.

Responsible for monitoring the clients’ treatment plan and progress towards goals; provides problem solving, conflict resolution and revision of goals and treatment plan as necessary.

Meets with Clinical Specialist/Director regularly to update and review client’s plan, progress, goals and schedule.

In consultation with the Clinical Specialist/Director, identifies, collaborates, and consults with physicians and other health care providers outside of the primary treatment team.

Works directly with Clinical Specialist, Director, Lead Brain Injury Specialist and other team members to design systems or strategies necessary to support the clients’ treatment plans.

Meets regularly with clients to provide support and guidance, to review goals and progress and to elicit and include client input on goals into treatment plans.

Serves as client advocate by communicating clients’ needs and issues to team members and other designated individuals; including plan for resolution and follow through as indicated.

Maintains regular contact with family members/support persons and provides orientation, education and updates as needed.

May support or provide direct care to clients at program locations or while on appointments as needed.

Monitors client programs in accordance with funding guidelines and constraints as advised by Marketing and Accounting, and shares necessary funding information with the team.

Ensures the financial integrity of client billing to include the completion of all necessary, related documentation. Provides information and documentation for additional or contracted services (e.g., added 1:1 hours for acute needs and equipment procurement).

Identifies and facilitates alternative and future sources of funding as needed (e.g., OVR, Medicare, Medicaid).

Maintains regular contact with funder and provides orientation, education and updates as needed.

Takes responsibility for seeking out current information regarding clients program and ReMed information (e.g., staff comm., rounds summaries, meeting minutes, client strategies, postings, memos, etc.).

Responsible for the clinical integrity of all clinical documentation through developing and revising log notes and strategies, etc.

Participates in problem solving with the team and supports the clinical team decision making process.

Complies with professional standards and state regulations regarding treatment.

Keeps current with professional certification/licensing requirements and participates in continuing education. Stays abreast of new developments, trends and treatment modalities in area of specialization.

Maintains thorough, accurate, and timely completion of required documentation. Writes monthly reports, monthly rounds summaries and contact sheets.

Monitors and ensures the completion and timely submission of all required documentation by designated staff.

Responsible for maintaining client charts according to established guidelines.

Prioritizes tasks to balance direct client treatment, staff communication, assigned program responsibilities and written documentation.

Qualifications

A bachelor’s degree along with case management and health care industry experience is required. A master’s degree and CRC or CCM designation is preferred. A valid driver's license, insurance and responsible driving behaviors must be maintained. Excellent planning, organizational, writing and time management skills are required. The ability to communicate with families, professionals and co-workers is a must. Strong clinical skills as shown by the ability to manage crisis situations, assess clients' needs and help formulate, prioritize and monitor treatment plans is necessary. The ability to demonstrate positive client interaction skills is essential.
