Job Description

Modified full-time (4 days per week) position working as a member of a multi-disciplinary team to provide assessment and education facilitating self-management to persons with diabetes.  This role works primarily in an outpatient setting with care occasionally delivered while a patient is admitted to hospital.  Patient populations include: pediatrics, persons with gestational diabetes or diabetes in pregnancy, adults with Type 1 diabetes.  Other duties as per job fact sheet.

 Skills and Qualifications:                

  • Four-year undergraduate degree from a Dietitians of Canada accredited university with a major in human nutrition (or equivalent) is required

  • Member in good standing with the College of Dietitians of Ontario is required

  • Member of Dietitians of Canada preferred

  • Certified Diabetes Educator designation strongly preferred; at minimum willingness to work towards

  • Experience with insulin therapy preferred; initiation and adjustment a definite asset

  • Experience with providing nutritional care to pediatric patients with diabetes an asset

  • Experience providing nutritional care to pregnant patients with diabetes an asset

  • Experience counseling persons with diabetes of varying ages and utilizing various treatment modalities would be an asset

  • Experience with pump therapy would be an asset      

  • Excellent interpersonal skills and ability to work as a member of an interdisciplinary team required

  • Experience with both individual and group counseling required

  • Excellent group facilitation skills required

  • Experience with Meditech would be an asset

  • Demonstrated ability to attend work on a regular basis is required
