
Nurse Practitioner, New Mexico
We are hiring for an innovative, collaborative, forward thinking heart failure APP to join the growing Biofourmis health technology-enabled services delivery company to launch our inaugural Heart Failure virtual specialty care product. You would manage growing panels of patients, and contribute to the growth and development of a telemedicine program that leverages first in class medication management titration algorithms combined with patient management principles.
Responsibilities:
Virtually evaluate patients entering the program and assist in creating a care plan based on patient specific needs and predesignated protocols
Respond to alerts escalated from health navigators related to patients’ deviations in vitals and physiologic data; symptoms; weight changes
Independently manage changes in clinical status that are within the APP’s scope of practice (e.g. dose change in diuretic after 3 pounds of weight gain over 24 hours)
Escalate out of scope clinical changes to lead MD
Liaise with healthcare navigators to ensure proper medications, labs and virtual appointments are ordered and communicated to the patient
Partner with registered nurses on staff to ensure that medications are reconciled on program entry and exit
Serve as the point of contact for each patient’s primary care teams, relaying information to them and tailoring this communication to individual providers’ needs
Provide education to patients about healthy behaviors and self-management
The ideal candidate will be:
A Board-Certified Nurse Practitioner with a minimum of 2 years of specialized heart failure experience in the ambulatory or inpatient setting
Previous tele health experience
Willingness to work in a changing, dynamic environment with a growing company
Able to work remotely
Motivated to provide health education and have a grounding in health promotion techniques
Able to work with health care providers at all levels and both non-medical and medical personnel
Proficiency with Windows-based computer applications, email and web-based skills.
Ability to learn and work in multiple EMRs
Must be able to work from home with a HIPPA compliant environment.
Must be available to work some holidays and weekends.
