We are looking for a hard-working social worker who is looking to make a difference in senior care!

Martin Luther Care Center, part of the Martin Luther Senior Living campus is located in East Bloomington, near woodlands/wetlands and along the Minnesota River Bluffs. Managed by Ebenezer, we offer 137 units between our Skilled Nursing, Memory Care and Transitional Care. We have an outstanding team of tenured staff! The only thing missing is YOU! The Licensed Social Worker helps senior residents and family understand and cope with the psychosocial aspects of their lives involving change, grief, and loss through crisis intervention - individual, group and family counseling.

Full Time Hours; 80 Hours Per Two Week Pay Period

Responsibilities/Job Description

Coordinates admissions with the admissions team as appropriate

Manages resident caseload

Provides one to one and group counseling as appropriate

Serves as a Discharge Planner

Assists in Vulnerable Adult investigations and completes necessary documentation and reports.

Qualifications

Bachelor's Degree from an accredited school of social work

Licensed by the MN State Board of Social Work
